Health Insurance
Despite the rising cost in healthcare costs, Axiom has elected to hold the employee premium at your current rate – this means you’ll continue to have your outstanding health insurance coverage and an extraordinarily low cost to you and your family!
- Single: $5 per pay period
- Family: $45 per pay period

Flexible Spending
We are happy to announce that we are adding the convenient debit card feature to your flexible spending plan in 2009!
ACTION: Even if you are a flexible spending participant, if you would like to enroll in flexible spending for 2009, you MUST complete a new flexible spending application. Please complete the following form and return to HR:

Vision Insurance

Axiom’s vision coverage is through United Healthcare (formerly Spectera), a leading vision provider that delivers affordable, innovative vision care solutions through a very diversified vision care network. You will have a choice of both private practice and retail chain providers.
Premiums remain the same for the 2009 plan year:
- Single: $3.97 per pay period
- Double: $6.93 per pay period
- Family: $10.49 per pay period
ACTION: If you do not wish to make any changes to your vision insurance plan, you do not need to take any action at this time. If you would like to make a change to your vision insurance plan, or enroll if you are not already a participant, please complete the following form and return to HR:

Dental Coverage
There has been a slight increase to dental premiums for the 2009 plan year. The rates still remain very competitive and we have worked to make our current dental plan even more comprehensive by adding orthodontics coverage!
Please note the new premium costs to you:
- Employee: $15.95 per pay period
- Employee + Children: $32.68 per pay period
- Employee + Spouse: $27.91 per pay period
- Family: $44.85 per pay period
